Individuals within this job classification are accountable for assessing, planning, implementing, evaluating and documenting patient related care activities with a focus on the CMS ta get diagnosis of AMI,CHF,COPD and pneumonia and transition management of these patients for 30 days after an acute care inpatient discharge. The transition Care Nurse-RN will be expected to adhere to and abide by the rules and regulations set forth by the Oklahoma Nursing Practice Act.

The Patient Care Coordinator will work with a multidisciplinary healthcare team to provide patient centered care across the health care continuum for Medicare (fee for service and Medicare advantage plan) patients. Will assist in the delivery of timely preventive care, care coordination and management across the medical neighborhood. Will collaborate with direct care nurses, hospitalists, case managers, patients, caregivers, PCP providers and providers care coordinators.

The Patient Care Coordinator will demonstrate the ability to interpret and convey complex information, using critical thinking skills. Will demonstrate effective, professional, tactful and appropriate, written verbal and non-verbal communication skills with patients and family members, medical staff, colleagues, other department and vendors, Will demonstrate office teaching techniques, applying adult learning principles motivational interviewing techniques. Will be proficient in use of MIDAS and hospital/provider EMRs. Will knowledgeable of workplace health and safety guidelines, preventive health maintenance, national evidence based guidelines, patient safety and case management.

High school diploma or GED, and current BLS certification and OK RN license required.

Position is located at 700 W. 7th Street.
